Timecard edit request

The timecard edit request form is for non-exempt realtime employees who record their time daily and use a terminal (time clock) or personal computer. Employees may submit this form when to request a timecard adjustment. This form is also required for non-worked hours such as vacation, sick, paid leave, comp time, leave without pay, etc. This is a helpful tool and document that can be used in a dispute. Forms need to be completed in ink, not pencil.
